The type of personal information we collect
We currently collect and process the following information:
• Personal identifiers, contacts and characteristics (for example, name, address and contact details) Next of kin details
•A full medical history and list of current medications, current ailments and list of treatments to date
Personal information is collected via a consultation form and is stored securely in a locked cabinet, access is restricted and can only be accessed by Gillian Bell Complementary Therapist.
All of the personal information we process is provided to us directly by you for one of the following reasons:
• 1.To provide a safe and appropriate treatment plan
We use the information that you have given us in order to –
Ascertain whether you are contra-indicated to the treatments
Note and act on any special care or adjustments needed for your safety and well-being during a treatment
Provide an individual treatment plan specific to your needs, requirements and if allergies are noted, ensuring the client has an epipen if required and any other medications required for their specific health conditions.
Use next of kin details in an emergency situation
Contact you with regards to any other relevant information pertaining to your appointment with Gillian Bell Complementary Therapist
Under the General Data Protection Regulation (GDPR), the lawful basis we rely on for processing this information are:
(a) Your consent to providing your medical history and any other relevant details required .You are able to remove your consent at any time. You can do this by contacting Gillian Bell on email gillbell1@btinternet.com
(c) We have a legal obligation to store your information in accordance with current GDPR ie safely, and securely. All information given is confidential and only accessed by Gillian Bell Complementary Therapist.
Information collected from you is via a consultation form , in a safe, secure, private and professional setting.
All information given is confidential and will not be shared unless in a life and death situation –

How we store your information
Your information is securely stored at Kinmylies Therapy Centre, Kinmylies Way, Inverness IV3 8TP in a secure, locked steel cabinet.
We keep consultation forms for 10 years We will then dispose of your information appropriately ie it will be securely destroyed.
